Please bring any and all medical records that pertain to any of the qualifying conditions listed above. You should bring your most recent medical records to your office visit. If you do not have any current medical records, our office can provide you with a release of information form to sign and we can obtain those records for you. We also offer PTSD screenings for an additional fee to those who may be unaware and suffering from this disorder.
Certification Fee:
- New Card/Patient: $200 (This is the total you pay initially)
- Renewal Card/Patient: $125 (This is only every 7 months by law)
- State Fee: $77.75 (Paid to the state yearly)
- PTSD Evaluation Fee (only if necessary): $60

One of our certifying physicians will enter your into the state of Florida’s medical use registry at your first visit. Currently the state of Florida is taking approximately 2 to 3 weeks to issue your physical medical use card. As their systems improve, this waiting period should be decreased.
No, insurance does not currently cover fees associated with the certification process or the medical use card fee.
